These FLORIDA-specific notarial certificates and forms are being provided for your convenience. All are statutory as referenced in the form description. If you are uncertain about which form or notarial certificate to use on your document, consult a licensed attorney. 

FLORIDA Notarial Certificates, Affidavits, and Blank Notarial Forms

Loose Certificates Download the form, fill it out and attach it to the document ONLY IF THERE IS NO EMBEDDED CERTIFICATE within the document or if you can not imprint it within the document. The loose certificates are the least desirable method of including a notarial certificate because of the susceptibility of fraud. Note: The FL Department of State strongly suggests that loose certificates not be used if there is another means because of how easily it can be misused.

If you HAVE TO use a loose certificate, make sure to add reference information to the loose certificate which will associate it with the applicable document should it become detached. See the “Imprinted” option to incorporate the notarial certificate into the document for a better solution. (Recommended)

Imprinted Download the form and print on a blank sheet, or print directly to your document such that the notarial certificate appears at the bottom of a page with adequate space (correct side up). This will embed the certificate into your document. The notary then will take the signer’s acknowledgment/oath, fill out the certificate, sign and apply their notarial seal.
Affidavits/Forms Download and print. Fill out the form, which is self-contained. Watch for the correct type of notarial certificate at the bottom.

ACKNOWLEDGMENTS (ACK) 

Typically used when you need to have your signature ‘verified’ to be from the person named in the document, OR, if you are agreeing to some conditions or actions that you have done or will be required to do. Representative examples might include personal letters, deeds, powers of attorney, contracts, mortgages, and last will and testament. 

JURATS (JUR) 

Typically used if you need to provide a sworn statement attesting to facts as presented in the document. The document MUST be complete (fields filled in) before the sworn statement is given and then signed in the presence of the notary. Representative examples might include affidavits, depositions, passport applications, and some applications. 

Single Signer (Signing in an Individual Capacity) 

1 Signer – Loose Certificate [ACK] [JUR] 

A JUR must be signed in front of a notary. Print this on a separate piece of paper if not already embedded in the document. Ref: [F.S. §117.05(13)(a)] [F.S. §695.25(1), §117.05(13)(b)] 

1 Signer – Imprinted [ACK] [JUR]

Two Signers (Signing in an Individual Capacity) 

2 Signers – Loose Certificate [ACK] [JUR] 

Both signers have their signatures notarized on the same day and in the same county. In the case of the JUR, both must sign in front of a notary. Ref: [F.S. §695.25(1), §117.05(13)(b)]; [F.S. §117.05(13)(a)] 

2 Signers – Imprinted [ACK] [JUR]

Representative Capacity (Signing on Behalf of Another) 

By Mark – Loose Certificate [ACK] [JUR]

This loose acknowledgment certificate is sufficient for the purpose of notarizing for a person who signs with a mark. Ref: [F.S. §117.05(14)(c)(2)]; [F.S. §117.05(14)(c)(1)] 

By Notary – Loose Certificate [ACK] [JUR]

This loose certificate is sufficient for the purpose of notarizing for a person with a disability who directs the notary to sign his or her name. Ref: [F.S. §117.05(14)(e)(2)]; [F.S. §117.05(14)(e)(1)] 

2 Credible Witnesses – Loose Certificate [ACK] [JUR]

Use this acknowledgment in conjunction with the FL 2 Credible Witnesses Affidavit for taking an acknowledgment or oath from a person without a statutory ID. [F.S. §117.05(5)(b)1, §117.05(13)(a)], §117.05(13)(b)]

Put the last page of your document back in your printer and then print this notarial certificate so it appears at the bottom of that page. Have notary take attorney in fact’s acknowledgment, fill out the information on the certificate, sign, and seal. [F.S. §695.25(5)] 

Corporate Officer – Loose Certificate [ACK] [JUR] 

Print out this loose certificate, staple to end of document, and have the signature of the corporate officer notarized. For the JUR, the signer must sign in your presence and under oath. [F.S. §695.25(2)]

Physician’s Affidavit (2191-AFF) 

Affidavit made by primary care physician that a principal lacks the capacity to manage property as defined in F.S. 744.102(12)(a). [F.S. §709.08(4)(d)] 

Affidavit for Law Enforcement Officers to attest to the confiscation of an inmate ID pursuant to FL Statutes. [F.S. §117.05(5)(b)2.h] 

Use this affidavit when giving a deposition oath and the court requests a notarized affidavit when identifying the deponent.

Online application for Florida Birth, Marriage, Dissolution of Marriage and Death Record from Vital Records Office suitable for Apostille or Authentication Certification. Certified copy will be created at the Bureau of Vital Statistics once input in received and identifying documentation is validated and your application will be sent to the FL Dept of State for Apostille issuance.

Application to Release a Florida Birth Certificate to a third party. Download, print and fill in form. Requires requestor’s ID, oath be taken and signed in the presence of a notary.

Application to Release Death Certificate with Cause of Death to a third person. Download, print and fill in form. Requires oath be taken of requestor and notarized signature (Jurat) in the presence of a notary.
