These FLORIDA-specific notarial certificates and forms are being provided for your convenience. All are statutory as referenced in the form description. If you are uncertain about which form or notarial certificate to use on your document, consult a licensed attorney.

Loose Certificates

Download the form, fill it out and attach it to the document ONLY IF THERE IS NO EMBEDDED CERTIFICATE within the document or if you can not imprint it within the document. The loose certificates are the least desirable method of including a notarial certificate because of the susceptibility of fraud. Note: The FL Department of State strongly suggests that loose certificates not be used it there is another means because of how easily it can be misused.

If you HAVE TO use a loose certificate, make sure to add reference information to the loose certificate which will associate it with the applicable document should it become detached. See the "Imprinted" option to incorporate the notarial certificate into the document for a better solution. (Recommended)
Imprinted Download the form and print on a blank sheet, or print directly to your document such that the notarial certificate appears at the bottom of a page with adequate space (correct side up). This will embed the certificate into your document. The notary then will take the signer's acknowledgment/oath, fill out the certificate, sign and apply their notarial seal.
Affidavits/Forms Download and print. Fill out the form, which is self-contained. Watch for the correct type of notarial certificate at the bottom.

JURATS ACKNOWLEDGMENTS
Typically used if you need to provide a sworn statement attesting to facts as presented in the document. The document MUST be complete (fields filled in) before the sworn statement is given and then signed in the presence of the notary. Representative examples might include: Affidavits, Depositions, Passport applications, some Applications. Typically used when you need to have your signature 'verified' to be from the person named in the document, OR, if you are agreeing to some conditions or actions that you have done or will be required to do. Representative examples might include: Personal Letters, Deeds, Powers of Attorney, Contracts, Mortgages, Last Will & Testament.

Both signers have their signatures notarized on the same day and in the same county. [F.S. §695.25(1), §117.05(13)(b)]

2 Signers - Imprinted (2027-JUR)
Can be used if two signers sign in front of a notary on the same date and in the same county. [F.S. §117.05(13)(a)]
2 Signers - Imprinted (2025-ACK)
This notarial certificate is used to notarize two signatures on the same date and in the same county. [F.S. §695.25(1), §117.05(13)(b)]

2 Notarial Certificates - Imprinted (2037-JUR)
Can be used if two signers sign in front of a notary on two different dates and/or in two different counties. [F.S. §117.05(13)(a)]
2 Notarial Certificates - Imprinted (2035-ACK)
Used if two signers have their signatures notarized on different dates and/or in different counties. [F.S. §117.05(13)(b)]


Representative Capacity (Signing on behalf of another)

By Mark - Loose Certificate (2113-JUR)
This loose jurat certificate is sufficient for the purpose of notarizing for a person who signs with a mark. [F.S. §117.05(14)(c)(1)]

By Mark - Loose Certificate (2111-ACK)
This loose acknowledgment certificate is sufficient for the purpose of notarizing for a person who signs with a mark. [F.S. §117.05(14)(c)(2)]
By Notary - Loose Certificate (2093-JUR)
This loose jurat certificate is sufficient for the purpose of notarizing for a person with a disability who directs the notary to sign his or her name. [F.S. §117.05(14)(e)(1)]

By Notary - Loose Certificate (2091-ACK)

2 Credible Witnesses (2145-JUR)
Use this jurat in conjunction with the FL 2 Credible Witnesses Affidavit for taking an oath from a person without a statutory ID. [F.S. §117.05(5)(b)1, §117.05(13)(a)]

2 Credible Witnesses - Loose Certificate (2143-ACK)
Use this acknowledgment in conjunction with the FL 2 Credible Witnesses Affidavit for taking an acknowlegment from a person without a statutory ID. [F.S. §117.05(5)(b)1, §117.05(13)(b)]

[There is no jurat example in FL statutes that suggests that an Attorney-in-Fact can swear an oath on behalf of a principle]

Attorney in Fact - Loose Certificate (2077-ACK)
Print out this blank acknowledgment, staple to the end of your document and have your signature as agent for the principle, notarized. [F.S. §695.25(5)]

[There is no jurat example in FL statutes that suggests that an Attorney-in-Fact can swear an oath on behalf of a principle]

Attorney in Fact - Imprinted (2071-ACK)
Put the last page of your document back in your printer and then print this notarial certificate so it appears at the bottom of that page. Have notary take attorney in fact's acknowledgment, fill out the information on the certificate, sign and seal. [F.S. §695.25(5)]

Corporate Officer - Loose Certificate (2055-JUR)
Print out this blank jurat, handwrite your information and have the signature of the corporate officer notarized in front of the notary. [F.S. §695.25(2)]

Corporate Officer - Loose Certificate (2051-ACK)

Partnership - Loose Certificate (2067-JUR)
Use this certificate to document the fact that you took an oath from the signer and that you witnessed them signing as a partner (or agent) of a partnership. [F.S. §117.05(13)(a)]

Partnership - Imprinted (2105-JUR)
Print your document, leaving adequate space at the bottom for this certificate. Reinsert your document in the printer and print this notarial certificate so it appears at the bottom of your document. Partnership - Imprinted (2061-ACK)
Print your document, leaving adequate space at the bottom for this certificate. Reinsert your document in the printer and print this notarial certificate so it appears at the bottom of your document. General Affidavit (Jurat) - 1 Signer (2007-AFF)

General Affidavit (Jurat) - 2 Signers (2027-AFF)

1 Credible Witness Affidavit (2131-AFF)
This affidavit is used when the signer does not have ID and the credible witness is personally known to both the signer and the notary. WARNING: ID would have to be difficult or impossible to obtain. [F.S. §117.05(5)(b)1]

2 Credible Witnesses Affidavit (2141-AFF)
This affidavit is used when the signer does not have ID, the credible witnesses personally know the signer and their identities are proved to the notary. WARNING: ID would have to be difficult or impossible to obtain. [F.S. §117.05(5)(b)1]

Attorney-In-Fact Affidavit (2075-AFF)
This is a loose certificate for the principle of an attorney-in-fact as requested by third parties. [F.S. §709.08(4)(c)]

Physician's Affidavit (2191-AFF)
Affidavit made by primary care physician that a principle lacks the capacity to manage property as defined in F.S. 744.102(12)(a). [F.S. §709.08(4)(d)]

Translation Affidavit (2201-AFF)
Affidavit on which the signature of a translator can be notarized where the translator certifies and swears to the accuracy of his or her translation. [FL Gov's Ref Man for Notaries 2001, p64]

Confiscation of Identification Affidavit (2161-AFF)
Affidavit for Law Enforcement Officers to attest to the confiscation of an inmate ID pursuant to FL Statutes. [F.S. §117.05(5)(b)2.h]

Self Proof Affidavit (2211-AFF)
For use with Last Will and Testament. Consult your attorney for relevant application. [F.S. §732.503]

Deposition Oath Affidavit (2221-AFF)
Use this affidavit when giving a deposition oath and the court requests a notarized affidavit when identifying the deponent.

FL Birth, Marriage, Dissolution and Death Certificate Application
Online application for Florida Birth, Marriage, Dissolution of Marriage and Death Record from Vital Records Office suitable for Apostille or Authentication Certification. Certified copy will be created at the Bureau of Vital Statistics once input in received and identifying documentation is validated and your application will be sent to the FL Dept of State for Apostille issuance.

Affidavit to Release Birth Certificate (DH 1958)
Application to Release a Florida Birth Certificate to a third party. Download, print and fill in form. Requires requestor's ID, oath be taken and signed in the presence of a notary.

FL VIN and Odometer Verification Form (HSMV 82042)
You can fill this form out on the computer and then print to have verified and certified by a FL notary.
Affidavit to Release Death Certificate with Cause of Death (DH 1959)
Application to Release Death Certificate with Cause of Death to a third person. Download, print and fill in form. Requires oath be taken of requestor and notarized signature (Jurat) in the presence of a notary.
FDLE Criminal Background Check Form
Request electronic results of your background check and make sure you request a "Certified Search" (NOTARIZED SIGNATURE) and have it sent to our attention. Mail required fee (fingerprints suggested for accuracy of search results) to FDLE address on form. Once we receive the results, we will have it Apostilled.

Federal Forms

DHS Form I-9, Employment Eligibility Verification (I-9)
Also the M-274 manual which provides additional information about the I-9. The employee signature on this form can be notarized with a Florida "loose certificate" attached. We DO NOT fill out or notarize employee signatures on I-9 forms.
FBI Identity History Summary ("Background Check")
See instrucations here. Print and fill in form. Have fingerprints taken at local law enforcement agency. You can have results mailed directly to us for Apostille since they will, by default, be "certified" by the agency.

Warning About Loose Certificates: Preprinted notarial certificates designed to be attached to a document should be used only in rare circumstances. Most documents will have a notarial certificate already printed on the document. Use that certificate, but make it comply with Florida law, if necessary. If the document has no notarial certificate, you should ask the document signer which notarial act is required for the execution of the document (an oath/affirmation or an acknowledgment). At the signer's direction, you may type or print the appropriate certificate on the document below the designated signature line for the document signer. ONLY IN RARE CIRCUMSTANCES SHOULD YOU ACTUALLY ATTACH A "LOOSE CERTIFICATE". If you do, be sure to state in the notarial certificate the exact document and signature to which the notarization applies.
